Transitional Services
Promoting success after Pathway
Parents and guardians play an integral role as a Pathway School student begins the transition from high school to the adult world. In addition to working closely with the family to develop an appropriate Transition Plan, we also encourage families to participate in Transition planning workshops, planning meetings, and events offered or suggested by The Pathway School’s Transition Services team.
The Pathway School’s Transition Coordinator supports students ages 14 and older with personalized transition planning for post-high school education/training, employment and independent living goals. As students advance through their academic and employment support programs, they spend more time meeting with their Transition Coordinator to review topics, which may include:
- Goal setting and self-advocacy
- Accessing adult services such as job coaching, habilitation support or job training and employment support
- Researching academic options for continuing education after Pathway
- Parents are encouraged to contact their child’s Transition Coordinator by email or phone with ideas, insight, questions, and concerns about preparing for success after graduating from The Pathway School.
