Transition Services
Transitions are a part of life for everyone. For the students (and family members of students) at The Pathway School, transitions provoke anxiety. The Administrative Staff at The Pathway School recognize the importance of thinking about “life after Pathway.” From this ideal, they developed a Transition Program to assist youth (15 years of age and older) and families map out plans. Among some of the services offered by the Transition Program are:

Assist youth/families understand the difference between the entitled child service world, and the eligibility world of the adult services |

Assist youth/families develop a better understanding of adult agencies that may be appropriate for assistance, funding and resources |

Assist youth identify the skills they will need to reach their long-term goals |

Work with the entire team at The Pathway School to plan for life immediately upon graduation and beyond |
 |
Provide families with a Transition Handbook, which includes general “transition” information specific to the state in which they reside |
 |
Work with the students to develop a Student Notebook which contains information they may find useful in their adult life |
 |
Provide information about various transition related topics through workshops hosted at The Pathway School and/or in the community |
Outside adult agencies are key to many successful transitions. The Transition Coordinator works with the parents to determine who the adult agencies are, and has a representative from such agencies included in the planning meetings in advance of graduation. Parents/guardians are encouraged to maintain close contact with adult agency representatives before and after planning. Because the adult service agencies rely on Governmental Funding in most situations, it is important that families begin the process of applying for services and discussing plans well in advance.
In some cases, youth/families are interested in continuing education/training. Many post-secondary training programs for students with disabilities include vocational programming, residential living, and daily living skills programs. Students and parents are encouraged to investigate as many options as possible in order to determine the best match for them. Pathway’s Transition Resource Library houses Post-secondary Placement options in addition to general information about Transition related issues.
