



Pathway's Transition Program has been developed around a person-centered approach and includes much more than simply attending the IEP (Individualized Education Plan) meeting. Our goal is to offer personal guidance to your son or daughter and to you throughout the years leading to his/her graduation.
The Process:
We schedule individual meetings with each student to discuss their goals and dreams for life after high school. During our Transition Planning sessions, we assist students in assessing their levels of independence, direct them to resources that may be helpful to them as a young adult, and help them explore how they can reach their transition goals. Whether their plan is to go to work with or without support, or to attend a post-secondary educational or vocational training program, we will encourage each student to find their "voice" and to understand that asking for assistance when it is needed is smart and mature.
Your child's assigned Transition Coordinator will join his or her IEP (Individualized Education Plan) team during the school year your child turns age 14. By working together for several years, we have the opportunity to work as a team to help identify your child's future goals and dreams, and to brainstorm ways to help reach those goals, through opportunities within Pathway and by accessing appropriate services outside of Pathway.
Transition Services become more individualized and frequent as students approach graduation. During the last two years at Pathway, each student will meet with their Transition Coordinator more often and will develop their own Transition Guide and timeline, with appropriate assistance. Seniors participate in "Senior Seminar" with interactive weekly lessons/discussions presented by a variety of speakers on topics such as self-advocacy, accessing accommodations in college, using public transportation, strategies to maintain wellness, and recreational/social opportunities.
You are extremely important to transition planning. We will schedule meetings or phone conferences with you prior to each IEP and encourage you to contact us by email or by phone with your ideas, insight, questions and concerns. We will listen and advise; at times we may also ask that you consider several options for your child. We will direct you to as many resources as we can, but, in the end, you and your child will be responsible for evaluating and accessing resources that best support your transition plans. We are here to guide you as you navigate the transition maze and prepare your child to enter the adult world.
Navigating the transition from the world of educational entitlement to the adult world of eligibility can be overwhelming and confusing. The Pathway School staff members have developed a strong network of resources which the Transition Coordinators will help you access.
